Minor, Major, Junior, & Senior teams
are required to fill out the following
forms to be handed into your coaches.
PROOF OF AGE
For Quebec, a photocopy
of a Medicare Card or Passport can be used as proof
of date of birth. However, only a Photocopy
of
the ORIGINAL BIRTH
CERTIFICATE is used to determine proof of age for tournaments at
the National and World Series level (Major, Junior,
& Senior).

PROOF OF RESIDENCY DOCUMENTS
Teams at the Major, Junior,
Senior levels require parents to provide THREE
proofs of residence. These documents need to be
DATED BEFORE FEBRUARY 1st. An envelope will be given
to each player to place your photocopies of your
proof of residence.
